I have read and understand the Regulated Tree Maintenance Rebate Guidelines and accept and abide by those terms and conditions 
I understand that any works may require separate development approval and I am responsible for gaining necessary approvals 
I understand that any information contained in this application (for example property location, work details and cost) will be made available to Council and the public as part of the corporate reporting process. 
Work was carried out by a qualified arborist and did not cause any tree damage
